Diabetes Intervention Program

Exercise assists in the control of type 2 diabetes by non-medical means, reduce the severity of the disease, and significantly reduce the risk of long-term complications.

The Type 2 Diabetes Program is designed to provide specialised individual and group treatment services to patients who have Type 2 diabetes.

There are two (2) elements to the provision of services under this scheme:

  • An initial assessment of individual patients; and
  • The provision of group class services

The purpose of the initial assessment is to prepare the patient for an appropriate group intervention. Eligible patients are able to claim one (1) Medicare rebate for their initial assessment for group class services; and eight (8) Medicare bulkbilled sessions for their type 2 diabetes group classes per year.

What is diabetes?

Diabetes mellitus is a condition where the body has trouble taking glucose from the blood and delivering it to the rest of the body so that it can be used as energy. This is due to lack of, or an inability to use insulin, the hormone required to “escort” glucose from the blood to cells of the body.

How exercise can help?

Aerobic exercise increases insulin sensitivity and, along with correct nutrition, helps restore normal glucose metabolism.

Strength training (resistance or weight training) also assists by increasing muscle tissue and increasing glucose uptake by the muscles thus enhancing the ability to store glucose.

Exercise can mean the difference between “medical management” and “lifestyle management” of Type 2 diabetes.

Exercise helps control Type 2 Diabetes by:

  • Decreasing Blood Sugar Levels
  • Decreasing Body Fat
  • Decreasing Waist Circumference
  • Increasing Insulin Sensitivity
  • Increasing Glucose Uptake
  • Increasing Energy Levels
  • Decreasing Cardio Vascular Health Risk

Group Diabetes Intervention Program

AAP has introduced the Group Diabetes Intervention Program to equip you with the knowledge, confidence and tools to help manage your diabetes. The group class utilises specialised equipment and the expertise and experience of an Accredited Exercise Physiologist (AEP) to implement tailored, patient specific treatment and service. This ensures continued motivation and adherence and eliminates physical restrictions and mobility issue barriers, allowing you to improve your health and well being through increased insulin sensitivity and glucose uptake.

If you have Type 2 Diabetes and would like to participate in our Group Diabetes Intervention Program you must have a current GP Management Plan in place. It is best to discuss this service with your GP so they can forward us the appropriate form.

General Practitioners / Practice Nurses:
If you would like to submit a diabetes group referral to AAP, we can supply you with an electronic form that is compatible with your practice software. We can fax, email or send a copy of the referral to you. Or it can be down loaded from here >>
