Patient’s referred via a Medicare TEAM CARE ARRANGEMENT
AAP’s team of Accredited Exercise Physiologists provide patients with exercise options and advice to assist the management of their chronic condition. Chronic conditions such as type 2 diabetes, ischaemic heart disease, osteoarthritis, stroke, chronic fatigue and COPD are examples of conditions that AAP can assist in the management of.
Patients referred to AAP via a GP Management Plan (GPMP), with a valid Enhanced Primary Care (EPC) referral will be bulk billed for the services that are conducted from AAP clinic locations for the number of visits specified on the EPC referral. Home visits and regional services incur a small gap payment.
Patients referred via Medicare TYPE 2 DIABETES referral
In addition to the visits provided under a Team Care Arrangement, patients with type 2 diabetes are also eligible to participate in an eight week group exercise and education program. AAP’s group diabetes exercise and education programs are conducted from our Preston location and are bulk billed with a valid Medicare Type 2 Diabetes Program referral.
To be eligible to receive a Medicare rebate for these services, patients will need to be referred by their GP and have their health managed through an Enhanced Primary Care plan.
